Diabetes Education and Support 

Albury Wodonga Health offers a comprehensive range of support to address the various needs of individuals living with diabetes within our region.

Our Credentialled Diabetes Educators (CDEs) provide diabetes support and education services for those living with Diabetes in a range of settings, including individual appointments, small group education and hospital base care.

We can help you with:

• Professional and expert advice from our trained team members
   
• Collaborative diabetes care with your GPs in the management of Type 1 Diabetes in children and adults, Insulin Pump therapy, continuous glucose monitoring, Type 2 Diabetes, and Diabetes in pregnancy
   
• Shared consultations with other members of the multidisciplinary team such as dietitians and podiatrists, the complex care team and specialist medical care (accessed via additional referral)
   
• Small group and individual consultations for expectant mothers with Gestational Diabetes

Who can access Diabetes Care?

Services are available to AWH hospital patients (inpatient), patients within our care - but not admitted to hospital (outpatient), and members of the community with diabetes with suitable referral*

• Paediatric Diabetes Clinic

Interpreter Service

This service is available upon request to clients from non-English speaking background to have information and consultation in the language of their choice.
